Statements from front-line experts in the field of intensive care are urgently needed. Sixteen front-line experts in China fighting against the COVID epidemic in Wuhan were organized to develop an expert statement after 5 rounds of expert seminars and discussions to provide trustworthy recommendation on the management of critically ill COVID patients. Each expert was assigned tasks within their field of expertise to provide draft statements and rationale.
Raised blood pressure BP is common after stroke but its causes, effects, and management still remain uncertain. Trials were identified from searching three electronic databases Medline, Cochrane Library and Web of Science Database. Three trials involving patients were included. Significant difference in BP values between treatment and placebo was found in two studies. No effect of the treatment was seen on dependency, death and vascular events at one, three or six months; the cumulative mortality and the number of vascular events at 12 months differed significantly in favour of treatment in one small trial which stopped prematurely. Evidence raises doubts over the hypothesis of a specific effect of ARBs on short- and medium-term outcomes of stroke.

The article discusses the causes of increased and decreased ratio as well as the clinical application of the ratio. The main focus of the first article  was physiological topics, such as urea production and renal processing of urea. The main focus of this second article is the clinical value of simultaneous measurement of urea and creatinine and calculation of the urea: creatinine ratio. The causes of increased and decreased ratio will be outlined and clinical application of the ratio will be discussed. The article begins with a short discussion of the comparative merits of urea and creatinine measurement for assessment of renal function; this discussion is helpful for an understanding of the potential clinical value of calculating the ratio. Irrespective of its cause, kidney disease is associated with decrease in GFR, and the severity of kidney disease correlates closely but inversely with GFR.
